Please complete the questionnaire if you wish to apply for membership of the association.

The questionnaire can be downloaded as a Word document or completed online.

There are three classes of Membership:

The information you give will assist the Association’s Board of Directors to decide on your suitability for appointment as a member in accordance with the association’s by-laws.
