The questionnaire is divided into 3 sections
1. General Information
2. Facilities
3. Specialised Marine Services (Parts A to G)

The information you give will assist the Association's Board of Directors to decide on your suitability for appointment as a member in accordance with the association's by-laws

Section 1 on General Information and Section 2 on Facilities must be answered fully

The work that you carry out may possibly cover an individual section or a combination of Sections 3A to 3G

Please complete whatever section or sections are applicable to your activity
